Abstract

intro_obj: El objetivo del presente trabajo es determinar si la aplicación de colgajos libres en la cobertura de los grandes defectos de la extremidad inferior ofrece ventajas sobre los colgajos pediculados y valorar como la etiología de la lesión, el momento de la intervención y el tipo de colgajo elegido condicionan el éxito de tratamiento. Se estudian 95 pacientes en los que se han realizado un total de 100 colgajos para la reconstrucción de defectos con exposición de estructuras nobles, focos de fractura etc. practicándose colgajos pediculados (79 % ), o colgajos libres vascularizados (21 % ). La etiología de los defectos fue: traumátismos 53 %, infecciosos 24%, tumores 7%, úlceras vasculares 8% y quemaduras 8%. El 60% de las lesiones estaban en tercio inferior de la pierna y pie. Valoramos los resultados según la necesidad de reintervención y las complicaciones menores y mayores. El índice de complicaciones menores es mayor en el grupo de los colgajos pediculados a expensas de pequeñas complicaciones en la zona donante. En los colgajos libres el índice de complicaciones tanto menores como mayores es menor . Sobre la base de estos resultados concluimos que la calidad de la reconstrucción se ve mejorada con la aplicación de los colgajos libres, influyendo decisivamente la severidad de la lesión y el momento de la intervención en el caso de los de origen traumático.
