Secuelas endocrinológicas del cáncer

Abstract

Los avances en el tratamiento del cáncer infantil han incrementado la supervivencia global al 75-80 %, llevando a un aumento en los efectos tardíos, como las endocrinopatías. Entre el 40 % y el 60 % de los supervivientes de cáncer infantil desarrollarán, al menos, una alteración endocrina. Estas secuelas son más frecuentes en supervivientes de tumores del sistema nervioso central, leucemias y linfomas. La radioterapia, especialmente la craneal, y el trasplante de progenitores hematopoyéticos (TPH) son los tratamientos con mayor riesgo de causar secuelas endocrinas. La radioterapia craneal puede dañar el eje hipotálamo-hipofisario, resultando en deficiencias hormonales, como el déficit de la hormona de crecimiento. El TPH, por otro lado, puede causar hipogonadismo, talla baja y obesidad. La quimioterapia también contribuye, con agentes alquilantes, causando disfunción gonadal a largo plazo. Las guías internacionales recomiendan un seguimiento endocrinológico de por vida para los supervivientes de cáncer infantil. Se debe realizar un cribado regular para detectar deficiencias hormonales, disfunción tiroidea, problemas de crecimiento, obesidad y sus comorbilidades, así como alteración en la salud ósea. El diagnóstico precoz y el tratamiento oportuno de las endocrinopatías son cruciales para mejorar la calidad de vida de estos pacientes a largo plazo.
