Abstract

El hiperandrogenismo es una situación de producción y/o acción androgénica excesiva. Se presenta con diferentes síntomas y signos a lo largo de la vida, dependiendo de la causa, de la edad y el sexo de los pacientes y de que haya otros problemas hormonales asociados. En las recién nacidas, se manifiesta en forma de genitales ambiguos; en la etapa prepuberal, de pubarquia o axilarquia prematura; en mujeres ya adolescentes y adultas, como hirsutismo, acné, alopecia androgénica e irregularidades menstruales, dando lugar a un síndrome de ovario poliquístico. La hiperplasia suprarrenal congénita es una de las causas más frecuentes de hiperandrogenismo en las distintas etapas de la vida. El tipo más común de hiperplasia suprarrenal congénita se debe a la deficiencia de 21-hidroxilasa. La forma clásica pierde sal (la más grave), poco frecuente, se caracteriza por hiponatremia, hiperpotasemia e hipotensión, siendo potencialmente mortal, y es la causa más común de genitales atípicos en neonatos con cariotipo 46,XX. Las formas no clásicas (leves) son más frecuentes que las clásicas y se manifiestan clínicamente por adrenarquia prematura, pseudopubertad precoz, hirsutismo y alteraciones menstruales.
