Abstract

La diabetes tipo 1 (DT1) es una enfermedad autoinmune crónica caracterizada por la destrucción progresiva de las células beta pancreáticas, que conduce a una deficiencia de insulina y representa una gran carga para los pacientes y sus familias. El objetivo del tratamiento es mantener la glucemia dentro de los rangos recomendados para prevenir complicaciones, proteger el desarrollo cerebral y mejorar la calidad de vida. En los últimos años, los avances tecnológicos han transformado profundamente el abordaje de la DT1. Los sistemas de infusión automatizada de insulina (AID) han demostrado una notable eficacia para optimizar el control glucémico, reducir el impacto psicosocial y mejorar la calidad de vida de niños, adolescentes y sus familias, convirtiéndose en el tratamiento de elección. Paralelamente, el diagnóstico precoz mediante la detección de autoanticuerpos permite anticipar la enfermedad antes de la aparición de síntomas clínicos, evitando su debut en forma de cetoacidosis y facilitando la participación en ensayos clínicos orientados a preservar la función pancreática y retrasar la progresión a estadios clínicos. Este artículo revisa los fundamentos del tratamiento actual, la importancia del diagnóstico precoz, las recomendaciones para el seguimiento en etapas preclínicas y las estrategias dirigidas a preservar la reserva funcional de células beta.
