Desequilibrio del Microbioma Humano desde la perspectiva causa–efecto

Abstract

Objetivo: Determinar si la capilaroscopía, aplicada mediante la metodología del Sistema de Aplicación de Técnicas para el Diagnóstico Metabólico (ATDM), identificaba en tiempo real los microorganismos que integran el microbioma humano y permitía su clasificación, con relevancia para el tamizaje preventivo y la toma de decisiones clínicas. Metodología: Se siguió un enfoque cuantitativo, documental y proposicional; primero se realizó una revisión bibliográfica en bases especializadas y luego una investigación clínica con bioimpedancia y capilaroscopía en n=6031 personas de ambos sexos y diversas edades en México, Estados Unidos, República Dominicana, Guatemala, Puerto Rico y Colombia; se construyó un atlas imagenológico y se aplicó reconocimiento de patrones morfológicos y cromáticos (modelo CIE-1931) para distinguir componentes por bioreflectancia y clasificarlos en tres indicadores: microbiota, macrobiota y micobiota. Resultados: La capilaroscopía identificó hallazgos compatibles con disbiosis bacterianas (p. ej., Staphylococcus aureus, Pseudomonas spp., Streptococcus spp.) y virales (chikungunya, Zika, flavivirus y SARS-CoV-2) en contextos clínicos controlados con lente de biofluorescencia; permitió reconocer macrobiota (protozoarios como Entamoeba histolytica, Giardia lamblia y Blepharisma; quistes y huevecillos de helmintos) y micobiota (p. ej., Trichophyton, Epidermophyton, Candida, Aspergillus, Coccidioides, Paecilomyces, Cryptococcus y hongos xilófagos), vinculando alteraciones tisulares y de microcirculación con patrones de simbiosis/disbiosis. Conclusiones: La capilaroscopía bajo el sistema ATDM constituyó una herramienta de bajo costo y alto impacto para identificar y clasificar microorganismos del microbioma en tiempo real. Tiene potencial para anticiparse a manifestaciones clínicas y orientar intervenciones oportunas y seguimiento. Se recomendó estandarizar protocolos, evaluar concordancia con pruebas de referencia y profundizar en la relación causa–efecto por grupos microbianos y escenarios de atención (consultorio, jornadas y telemedicina).
