Abstract

Introducción: La dermatitis atópica (DA) facial requiere antiinflamatorios tópicos eficaces y seguros que no comprometan la barrera cutánea delicada de cara y cuello. Los corticoesteroides tópicos de potencia media (CPM) son estándar, pero pueden inducir atrofia, telangiectasias y disrupción perioral. Los inhibidores tópicos de calcineurina (ITC: tacrolimus, pimecrolimus) ofrecen alternativa esteroide-sparing, si bien su eficacia relativa es motivo de debate (1–3). Objetivo: Comparar eficacia (EASI-50/IGA clear–almost clear) y seguridad de ITC frente a CPM en DA facial mediante revisión sistemática PRISMA y síntesis cuantitativa. Metodología: Se buscaron PubMed, Scopus y Web of Science (2000-abril 2025). De 1 026 registros, 28 estudios cumplieron inclusión (10 ECA, 8 cohortes, 10 series). Diez ensayos (n = 1 042) fueron elegibles para metaanálisis de efectos aleatorios. Resultados: ITC alcanzaron EASI-50 en 68 % (IC95 % 62-74) vs 72 % (IC95 % 65-78) con CPM; RR 0.95 (IC95 % 0.87-1.04). La tasa de IGA clear/almost clear fue equivalente (RR 0.97; IC95 % 0.88-1.06). ITC redujeron eventos de atrofia (0 % vs 6 %; RR 0.08, IC95 % 0.01-0.63) y telangiectasias (1 % vs 5 %; RR 0.22, IC95 % 0.07-0.70) (9–13). Ardor transitorio fue más frecuente con tacrolimus 0.1 % (21 % vs 6 %; RR 3.5, IC95 % 2.4-5.1). Conclusiones: Los ITC muestran eficacia clínica comparable a los CPM para DA facial y un perfil de seguridad superior en términos de atrofia y telangiectasias; el aumento de sensación de ardor es autolimitado y tolerable. Se recomiendan como primera línea en zonas faciales otras de piel fina, especialmente en uso crónico o pediátrico.
