Možganski edem: astrociti v patofiziologiji celičnega edema

Abstract

Možganski edem se pojavi pri različnih patologijah možganov (npr. možganske poškodbe, ishemije, okužbe) in lahko privede do hudih okvar možganov, tudi smrti. Zanj je značilno prekomerno kopičenje vode v znotrajceličnem in zunajceličnem prostoru, ki je posledica porušene ionske homeostaze, predvsem ionov Na+. Ta ustvarja osmotski gradient, ki v prvi stopnji edema usmerja tok vode iz zunajceličnega prostora v celice, kar povzroča nabrekanje celic (celični ali citotoksični edem). V kasnejših stopnjah edema pa osmotski gradient usmerja tok vode tudi iz žilja v zunajcelični prostor, kar vodi do nabrekanja možganskega tkiva (ionski, vazogeni in hemoragični edem). Homeostazo ionov in vode v možganih uravnavajo astrociti, ki so morfološko in funkcionalno izredno heterogene celice nevroglije. Zaradi visoke prepustnosti membrane za vodo, ki je posledica izražanja vodnih kanalov akvaporinov tipa 4, so astrociti prve celice v možganih, ki se ob porušeni ionski homeostazi odzovejo s privzemom vode, kar vodi v nabrekanje celic (celični edem). Celični edem nastane le nekaj minut po poškodbi možganov. V preglednem članku bomo predstavili vzroke za nastanek in značilnosti različnih stopenj možganskega edema s poudarkom na celičnem edemu. Doslej so bili v astrocitih prepoznani številni molekularni mehanizmi in beljakovine, ki so vpleteni v nastanek celičnega edema, med drugim od ATP odvisna Na+/K+-ATPaza, akvaporin tipa 4 in Na+/K+/2Cl–kotransporter tipa 1, vendar pa učinkovitih tarčnih terapij za zdravljenje celičnega edema ni. Zaradi svoje vloge pri nastanku celičnega edema astrociti predstavljajo pomembno tarčo za razvoj postopkov in zdravil, ki bi preprečili ali omilili nastanek celičnega edema in s tem ostalih stopenj možganskega edema ter z njimi povezanih zapletov.
