Radioterapija s protoni

Abstract

Radioterapija je zdravljenje bolezni z ionizirajočim sevanjem, ki povzroči okvaro genetskega zapisa celice in s tem onemogoči njeno delitev. V procesu zdravljenja raka jo lahko uporabljamo samostojno ali v kombinaciji z drugimi načini zdravljenja. Prednost obsevanja s protoni pred konvencionalnim obsevanjem z žarki X ali elektroni je v ugodnejši dozni porazdelitvi in večji radiobiološki učinkovitosti, kar se kaže v manjši dozni obremenitvi zdravih tkiv. Posledično je možno tumor obsevati z višjimi odmerki ionizirajočega sevanja, kar omogoča boljšo kontrolo tumorja. Radioterapija s protoni je v klinični uporabi zadnjih 30 let, vendarje zaradi zahtevne tehnologije in visoke cene dostopna v manjšem številu centrov kot konvencionalno obsevanje. Od obsevanja s protoni naj bi imelo klinično pomembne koristi do 20 % bolnikov, ki so napoteni na radioterapijo. Med indikacije za obsevanje s protoni sodijo tumorji, ki jih je zaradi njihove radioodpornosti treba obsevati z visokimi odmerki in je zato zdrava tkiva/organe v bližini treba učinkovito zaščititi. Večinoma pa bolnike napotujemo na tovrstno obsevanje z namenom zmanjšanja akutne in pozne toksičnosti in boljše kakovosti življenja. To je še posebno pomembno pri otrocih, pri katerih je pričakovana življenjska doba daljša in so za toksično delovanje obsevanja bolj občutljivi kot odrasli. Za zdaj slovenske bolnike na radioterapijo s protoni pošiljamo v tujino.
