Doble sistema colector bilateral congénito

Abstract

Introducción: El doble sistema colector es la anomalía congénita menos frecuente del tracto urinario superior, con una incidencia de 0.8%. Es más frecuente en la población femenina. Su diagnóstico prenatal es posible mediante ecografía, pero suele confirmarse tras el nacimiento cuando aparecen complicaciones. El objetivo del presente estudio fue reportar un caso de doble sistema colector bilateral en una recién nacida y analizar su relevancia clínica. Reporte de caso: Se describe un caso de neonato femenino de 38,5 semanas de gestación, con antecedentes de dilatación pielocalicial bilateral detectada en la ecografía prenatal. Posteriormente, presentó infecciones urinarias recurrentes. Taller diagnóstico: Los estudios postnatales mostraron hidronefrosis bilateral con duplicación pielocalicial completa en ambos riñones. La paciente presentó episodios de sepsis neonatal e infección urinaria por bacterias multirresistentes. Fue tratada quirúrgicamente mediante una anastomosis uretero-ureteral, técnica que permitió preservar la función renal y reducir las complicaciones. Discusión: La duplicación ureteral puede ser completa o incompleta y suele asociarse a hidronefrosis e infecciones recurrentes. El tratamiento quirúrgico está indicado en casos de obstrucción, de reflujo vesicoureteral o de infecciones de repetición. La anastomosis uretero-ureteral constituye una opción conservadora con menor morbilidad que el reimplante ureterovesical. Conclusiones: El diagnóstico prenatal de anomalías del tracto urinario permite un seguimiento adecuado y la reducción de complicaciones. La anastomosis uretero-ureteral es una alternativa segura y eficaz en pacientes pediátricos con doble sistema colector bilateral.
