Abstract

Introducción: El deterioro cognitivo leve (DCL), en Ecuador, podría estar subdiagnosticado, por la recomendación de la normativa oficial de utilizar el Mini-Mental State Examination (MMSE) de manera exclusiva para evaluar las funciones cognitivas de los adultos mayores. Este artículo expone la necesidad de protocolos diagnósticos más sensibles que permitan una detección temprana del DCL. Reporte de caso: Mujer de 85 años, con hipertensión e hipoacusia, bajo nivel educativo y quejas subjetivas de memoria progresiva desde hace 2 años, mantiene autonomía funcional; corroborada por familiares. Taller diagnóstico: El MMSE fue normal; el Neuropsi mostró alteraciones en la memoria y en las funciones ejecutivas, compatibles con DCL amnésico unidominio. Evolución: Síntomas lentos, sin pérdida de autonomía, contraste de resultados refleja el riesgo de subdiagnóstico por uso exclusivo del cribado. Conclusión: Se indica la necesidad de complementar los cribados recomendados por la normativa con evaluaciones neuropsicológicas para detectar DCL e intervenir de forma temprana, evitando la progresión hacia trastornos neurocognitivos mayores.
