Abstract

Introducción: La mediastinitis descendente es una complicación de infecciones polimicrobianas agudas orofaríngeas o cervicofaciales. Donde existe diseminación hacia el mediastino, a través del espacio localizado entre la región prevertebral y alar de la fascia cervical profunda. Es una entidad rara, con alta mortalidad (19-40%). Afecta principalmente a adultos jóvenes, a predominio de hombres. El objetivo de este póster es demostrar mediante imágenes la historia natural de la mediastinitis descendente en un paciente con un absceso periamígdalino. Caso clínico: Masculino de 64 años de edad, con antecedentes de diabetes mellitus tipo 2, hipertensión arterial e hipotiroidismo. Con síntomas de odinofagia, disfagia, trismus, fiebre, dorsalgia y dolor torácico de 3 semanas de evolución. Tratado con múltiples esquemas antibióticos, con evolución tórpida. Al ingreso el paciente presentaba halitosis , amígdalitis purulenta, cuello edematoso con eritema generalizado e inestabilidad hemodinámica por lo que se decide soporte ventilatorio. En base a los hallazgos tomográficos de mediatinitis y el deterioro clínico del paciente se optó por tratamiento quirúrgico y antibiótico terapia de amplio espectro, empírica. Se realizó amigdalectomía, drenaje de absceso periamigdalino izquierdo, cervicotomía laterocervical izquierda y submentoniana, toracotomía lateral, mediastinotomía y drenaje mediastinal, en tratamiento con vancomicina + meropenem de larga estancia en UCI con alta en 2 meses. Conclusiones: La tomografía con contraste intravenoso es determinante para el diagnóstico y la decisión de tratamiento más efectivo, ante la sospecha clínica de mediastinitis descendente. El tratamiento es controversial, concordando que en casos de abscesos y colecciones la cirugía podría estar indicada como terapia inicial. El inicio terapéutico oportuno mejora el pronóstico, mortalidad y morbilidad de los pacientes.
