Laminoplastia cervical en pacientes con estenosis de canal y mielopatía.

Abstract

Introducción: La estenosis espinal cervical provoca compresión medular, isquemia y cambios degenerativos de la misma, traduciéndose clínicamente por disfunción neurológica de extremidades superiores e inferiores con deterioro de la marcha. En la resonancia esta degeneración se observa hiperintenso a uno o varios niveles cervicales. La laminoplastia de puerta abierta es una técnica quirúrgica para estenosis multinivel (3 o más niveles) con signos de mielopatía y dolor axial en pacientes que conservan la lordosis cervical. Caso clínico: Hombre de 52 años, con cervicalgia de 2 meses luego de caída con impacto directo en región cervical posterior. Se acompaña de parestesia y debilidad muscular en extremidades superiores ((3/5) en territorio C5 y C6 izquierda) e inferiores (hiperreflexia rotuliana izquierda). Al momento con talla vesical, Nurick 4, MJOA 9, Signo de Hoffman +, Signo de Trommer +. En rx de columna cervical tiene cambios artrósicos avanzados. En la TAC con presencia de osteofitos y disminución del cana raquídeo. En Resonancia compresión medular a tres niveles C3-C4, C4-C5, C6-C7. El paciente fue sometido cirugía de laminectomía. En los resultados la incidencia de mielopatía fue del 7%. Se obtuvo resultados funcionales favorables mJOA de 9 prequirúrgico a 14 postquirúrgico, recuperando hasta el 50 % del canal espinal. Conclusión: La laminoplastía de puerta abierta es una opción efectiva y segura para el tratamiento de la estenosis espinal cervical multinivel, ofreciendo beneficios significativos en la descompresión de la médula espinal, mejorando la funcionalidad cervical y minimizando las complicaciones.
