Incontinencia pigmenti (síndrome de Bloch-Sulzberger) en la etapa neonatal

Abstract

Introducción: La Incontinencia Pigmenti (IP) es un trastorno neurocutáneo raro dado por la alteración hereditaria ligada al cromosoma X, se presenta un caso clínico. Reporte de caso: Niña recién nacida, quien es valorada a las 48 horas de vida, por lesiones en piel con vesículas eritematosas-ampollas pustulosas, ubicadas en el miembro inferior izquierdo, presentes desde el nacimiento. Las lesiones se extendieron aumentando en número a otras regiones, además de irritabilidad sostenida. Taller diagnóstico: Se realiza biopsia de piel y se diagnostica de incontinencia pigmenti. El estudio genético determinó una genodermatosis dominante ligada al cromosoma X, asociada a la mutación del GEN IKBKG. Estudios de control hematológico, químico y hormonal fueron normales. No existió compromiso neurológico u oftálmico. Evolución: La paciente, con lesiones en fase vesicular, recibió tratamiento tópico con óxido de zinc y calamina. En su evolución se agregaron lesiones tipo pápulas sobre infectadas, por lo que requirió oxacilina. Fue dada de alta en mejores condiciones, sigue en observación. Conclusiones: En el presente caso se plantean diagnósticos diferenciales como el impétigo ampollar, penfigoide ampolloso, herpes neonatal, citomegalovirus, mastocitosis, epidermólisis ampollar hereditaria o eritema tóxico.
