Arritmia por reentrada ventricular izquierda

Abstract

La taquicardia ventricular (TV) idiopática engloba un grupo heterogéneo de taquicardias ventriculares, en las cuales no se puede demostrar cardiopatía estructural (displasia del ventrículo derecho, el prolapso de la válvula mitral, ciertas miocardiopatías), alteraciones metabólico- electrolíticas, presencia de intervalo QT largo o síndrome de Brugada. En general la TV se considera idiopática en un 11%. Las TV idiopáticas fasciculares izquierdas son una forma infrecuente de TV, representan del 10 al 15 % siendo más frecuentes en adultos jóvenes (15 a 40 años), de los cuales entre 60% y 80% son hombres. La forma mayoritaria de estas taquicardias involucra al fascículo posterior y por ello se denomina TV fascicular posterior, con morfología de bloqueo de rama derecha y eje superior. Aunque la terapia farmacológica puede ser eficaz en un buen porcentaje de los pacientes, la ablación tiene una alta tasa de éxito, baja tasa de complicaciones y de recurrencia a largo plazo. Las tasas globales de éxito para la ablación de TV fascicular idiopática oscilan entre el 70% y el 90% en relación con el seguimiento a largo plazo. Se presenta un caso clínico en el congreso anual del Hospital Alcívar.
