PENFIGOIDE AMPOLLOSO EN PEDIATRÍA. A PROPÓSITO DE UN CASO

Abstract

El penfigoide ampolloso es una enfermedad autoinmune poco frecuente en edad pediátrica; caracterizada por la presencia de autoanticuerpos en la zona de la membrana basal epitelial. Ésta enfermedad se ha relacionado con la ingesta de fármacos, procesos autoinmunitarios y con administración de vacunas con la aparición de las lesiones hasta 4 semanas después de dicho factor predisponente. Su tratamiento se basa en medicamentos inmunosupresores y antiinflamatorios, teniendo así un pronóstico bueno, mejor que la variedad del paciente adulto.Presentamos el caso de un escolar masculino, que ingresa por presencia de lesiones ampollares en manosypies, con resultado de biopsia en primera instancia de eccema dishidrótico severo, recibiendo corticoide vía oral, curaciones por servicio de cirugía plástica y antibioticoterapia intravenosa, con notable mejoría de lesiones que a las dos semanas presenta una exacerbación del cuadro con la aparición de nuevas lesiones generalizadas a nivel de tronco, áreas flexurales, y regiones acrales de manos y pies caracterizada por ampollas grandes tensas, con contenido serosanguinolento, pruriginosas, dolorosas a la manipulación.
