Abstract

La diabetes en el embarazo constituye un conjunto de patologías de diferente diagnóstico y resultados perinatales que aportan, cada una, importante morbimortalidad materno fetal. Se ha demostrado que los factores de riesgo principales implican datosmaternos como la edad avanzada, el IMC elevado,la historia de diabetes gestacional, así como las complicaciones hipertensivas de gestas. A la vez, las consecuencias de estas patologías hiperglucémicas tienen claros resultados adversos materno-fetales, entre los cuales se mencionan la macrosomía fetal, hipoglucemia, prematuridad, malformaciones congénitas, síndrome de distrés respiratorio e incluso muerte fetal.OBJETIVO: Determinar los principales factores de riesgo, consecuencias y complicaciones ocasionados por la diabetes gestacional.Metodo: Estudio no experimental, de corte transversal, descriptivo y correlacional, en el Hospital de Especialidades José Carrasco Arteaga, durante el período abril 2019 a abril 2020.Resultados: En la presente serie se evidencia mayor prevalencia de diabetes pregestacional sobre la gestacional (62,5% versus 37,5%). En cuanto a los factores de riesgo, la mayor prevalencia de diabetes gestacional se encontró en el grupo de 36 a 40 años (41,1%); el diagnóstico nutricional más prevalente fue el sobrepeso. La complicación del producto más prevalente fue la hipoglucemia (60%) seguida por la macrosomía fetal (30%) y óbito fetal (10,58%) todos con mayor frecuencia en la diabetes gestacional que en la pregestacional. Las complicaciones obstétricas ocurrieron en el 50% de los casos, con predominio de la distocia por desproporción cefalopélvica en la diabetes gestacional (30,8%) y distocia por hipotonía uterina (18,5%) en la pregestacional.
