Aspectos endoscópicos e manométricos da motilidade esofágica ineficaz

Abstract

Introdução: A motilidade esofágica ineficaz é definida como um distúrbio menor da motilidade esofágica, e o mais frequente. A relação com a doença do refluxo gastroesofágico ainda é controversa. Objetivo: Identificar a doença do refluxo gastroesofágico, comparar e correlacionar os achados endoscópicos e manométricos na motilidade esofágica ineficaz. Método: Foram avaliados retrospectivamente 40 pacientes com diagnóstico manométrico de motilidade esofágica ineficaz, no período de agosto de 2020 a janeiro de 2023, no Hospital do Servidor Público Estadual “Francisco Morato de Oliveira” de São Paulo. Resultados: Dos 40 indivíduos, 10 (25%) tinham achados endoscópicos conclusivos para doença do refluxo gastroesofágico; 7 (17,5%) apresentavam esôfago de Barrett; 2 (5%) cursavam com esofagite erosiva distal grau D e 1 (2,5%) com grau C de Los Angeles. Destes 10, nove (90%) tinham mais de 80% de ondas ineficazes na manometria esofágica de alta resolução. Dos 30 pacientes em que a endoscopia não foi conclusiva para doença do refluxo gastroesofágico, 19 (63,3%) apresentavam refluxo patológico à pHmetria. Conclusão: O estudo encontrou uma alta correlação entre a motilidade esofágica ineficaz e a doença do refluxo gastroesofágico. Observou-se, nesta amostra, uma percentagem maior de ondas ineficazes, sugerindo que a gravidade de uma doença é diretamente proporcional à outra.
