Jetra i mozak u dijalogu - neurofiziološke posledice metabolopatija

Abstract

Nealkoholna masna bolest jetre obuhvata spektar oboljenja sa metaboličkom disfunkcijom kao glavnom odlikom, u koji spadaju masna jetra, steatohepatitis, ciroza i fibroza jetre, a predstavljeni su jednim terminom - MASLD (eng. Metabolic dysfunction-Associated Steatotic Liver Disease). MASLD prevazilazi lokalne hepatične poremećaje, već ima i dalekosežne sistemske posledice na sve organske sisteme, uključujući i centralni nervni sistem. Osovina jetra-mozak funkcioniše kao dinamičan dvosmerni sistem komunikacije posredstvom humoralnih i imunolo-ških medijatora. U uslovima metaboličke disfunkcije jetre, dolazi do dislipidemije, insulinske rezistencije, oksidativnog stresa i hronične niskostepene inflamacije, što pokre-će kaskadu događaja sa neurofiziološkim posledicama. Iako su kardiometaboličke komplikacije najčešća posledica MASLD, novija istraživanja sve više daju značaj potencijalnim neurološkim posledicama koje uključuju poremećaje kognitivnih funkcija, raspoloženja, neurodegenerativne i cerebrovaskularne bolesti. Hepatična i sistemska inflamacija, poremećena sinteza i metabolizam aminokiselina i disregulacija insulinskog odgovora utiču na neurotransmiterske sisteme i plastičnost sinapsi. Pored toga, mitohondrijalna disfunkcija i oksidativni stres u neuronima doprinose razvoju neuroinflamacije, dok promene u propustljivosti krvno-moždane barijere doprinose lakšem prolasku toksičnih metabolita u moždano tkivo. Kliničke manifestacije mogu varirati od suptilnih kognitivnih deficita i poremećaja pažnje i koncentracije, preko simptoma anksioznosti i depresije, pa sve do encefalopatija u uznapredovalim stadijumima. Razumevanje neurofizioloških posledica MASLD je od ključnog značaja za rano prepoznavanje ovih poremećaja i blagovremenu intervenciju u cilju sprečavanja njihovog razvoja i pogoršanja. Od posebnog je značaja korigovanje životnih navika u kombinaciji sa farmakološkim terapijama usmerenim na dislipidemiju, insulinsku rezistenciju i oksidativni stres. Sagledavanje MASLD iz ugla neurofiziologije naglašava potrebu za multidisciplinarnim strategijama koje objedinjuju hepatologiju, endokrinologiju i neurologiju u cilju očuvanja mentalnog zdravlja i prevencije kognitivnih poremećaja kod metabo-ličke disfunkcije jetre.
