Introducao As situacoes de instabilidade hemodinâmica durante a realizacao de coronariografia sao mais comuns em doentes com doenca coronaria grave, sobretudo na presenca de sindromes coronarias agudas. Raramente podem ocorrer em contexto eletivo, mesmo na ausencia de doenca coronaria, devido a embolizacao gasosa inadvertida e ao aumento da vasorreatividade coronaria1,2. O vasospasmo coronario tem sido implicado como um dos possiveis mecanismos fisiopatologicos envolvidos na miocardiopatia de Tako-tsubo (MCT), embora estejam descritos poucos casos na literatura que comprovem esta associacao3. Apresentamos o caso de um paciente com instabilidade hemodinâmica transitoria durante a realizacao de coronariografia eletiva, com evidencia de vasospasmo multivaso e achados sugestivos de MCT.
