
Clear aligner therapy has undergone remarkable advancements in recent years, transforming modern orthodontic practice through continuous technological and material innovations. Innovations in clear aligner therapy focus on improving treatment efficiency, accuracy, patient comfort, and the ability to manage complex malocclusions. The integration of digital technologies such as intraoral scanning, three-dimensional imaging, artificial intelligence, and CAD/CAM systems has enhanced diagnosis, virtual treatment planning, and customized aligner fabrication. Advanced thermoplastic and shape memory materials provide improved elasticity, force control, durability, and aligner adaptability. Innovations including optimized attachments, smart force systems, remote monitoring applications, and direct 3D-printed aligners have further increased the predictability and convenience of treatment. Additionally, the incorporation of smart sensors, teledentistry, and digital workflow systems has improved patient compliance and clinician-patient communication. Emerging technologies such as nanotechnology, bioactive materials, and artificial intelligence-driven treatment simulations continue to expand the scope of aligner therapy in managing complex orthodontic cases. Despite these advancements, challenges related to cost, long-term clinical validation, and treatment limitations still exist. This review highlights recent innovations, clinical applications, benefits, limitations, and future directions of clear aligner therapy in contemporary orthodontics.
