Este artículo analiza cómo las empresas sociales de salud utilizan el bricolaje social para enfrentar restricciones de recursos sin comprometer la calidad asistencial, la seguridad del paciente ni el cumplimiento regulatorio. A partir de un estudio cualitativo de casos múltiples con cuatro organizaciones del estado de São Paulo, basado en entrevistas en profundidad y análisis intercasos, se observa que el bricolaje social constituye un mecanismo central para recombinar recursos, movilizar actores y sostener la operación y expansión de estas iniciativas. Sin embargo, su uso no es irrestricto: se encuentra condicionado por exigencias técnicas, regulatorias y éticas propias del sector salud. Los hallazgos muestran que la improvisación y la superación de limitaciones son más frecuentes en etapas iniciales o en actividades de menor riesgo, mientras que la creación de valor social, la participación de stakeholders y la persuasión permanecen como componentes estructurales.
