Imprint cytology in pituitary adenoma

Abstract

目的 : 下垂体腫瘍に対し捺印細胞診を行い, 腺腫細胞の特徴を明らかにし, 術中迅速診断に有用であるか検討を行った.方法 : 下垂体腺腫摘出 94 例から術中迅速組織診検体を用いて捺印標本を作製し, その組織像と比較検討した.成績 : 下垂体腺腫 86 例 (91.5%) で下垂体腺腫の確認が可能であった. また, 組織亜型分類では, fibrous body や分泌顆粒などの特徴的な細胞内構造物に加え, 細胞形状, 細胞結合性, 核の位置などの細胞所見を総合的に捉えることによって, 3 群 (GH-PRL-TSH 群, ACTH 群, FSH-LH/ナルセル群) に分類可能であった.結論 : 下垂体腺腫の捺印細胞診は, 腺腫細胞の特徴を十分に捉えることが可能であり, 迅速診断のみならず組織診断を補完する有用な手法と考えられた.
