Abstract

目的:脳腫瘍のなかでも発生頻度の高い星細胞系腫瘍の捺印細胞所見を組織学的grade分類に対比させ, grade別の細胞所見をまとめて有用性を検討した.方法:材料は脳腫瘍迅速診断に提出された星細胞系腫瘍81例 (星細胞腫grade Iが6例, grade IIが28例, grade IIIが14例, grade IVが33例) の捺印細胞標本を用いた. 同一症例の組織標本と 対比し, 組織学的grade分類に対応する捺印細胞所見をgrade別にまとめた.成績:grade Iの細胞は毛様, 線維性で細胞質の保持が良い. grade IIでは細線維性基質上に類円形の核が並ぶ. grade IIIでは出現する集塊の細胞密度が高く, 軽度の核異型が認められる. grade IVでは巨細胞, 巨核・多核・多形性など高度の核異型と, 輪郭のはっきりした細胞質がみられる.結論:組織構築をみることに限界のある細胞診断はそれ独自では星細胞腫の確定診断となりにくいが, 迅速な標本作製が可能で細胞の全体像をみることに適しており, gradeの推定に有益な情報が得られることが明らかになった.
