Clinical significance of Ag-NORs-staining in multiple myeloma.

Abstract

本研究では多発性骨髄腫multiple myeloma: MMの骨髄標本におけるAg-NORs染色を試みた.MMのAg-NORs染色で得られたドットは, 数・大きさ・染色パターンに多様性があった.われわれは, ドットの染色パターンにより独自のscoringsystemを考案し, 臨床の場で応用されている病期分類, 形態分類と対比して検討を行った.このscoringsystemは, 病期分類に比し形態分i類とよく相関していた.また, totalscore≧300群は生存期間が有意に短く予後不良であった.totalscore≥300群は, ゆっくりした経過をとり化学療法なしに観察できる症例と思われた.経過を観察し得た症例では, 病勢を反映してAg-NORsの染色パターンが変化した.Ag-NORsは, MMの予後因子として, また治療選択の上からも重要な指標として臨床的意義は大きく, 本scoring systemは実用的なものであると考えられた.
