Abstract

Le zona est une maladie douloureuse provoquée par une réactivation du virus de la varicelle. Dans la plupart des cas, il guérit spontanément en 2 à 4 semaines. Il peut toutefois avoir des complications, comme la névralgie post-herpétique (10 à 20 % des cas de zona), ou d’autres complications plus rares mais aussi plus graves. Le zona touche plus souvent les personnes âgées et celles dont l’immunité est déficiente. Depuis 2021, un nouveau vaccin (Shingrix®) est arrivé sur le marché belge. Le Conseil supérieur de la Santé (CSS) a recommandé qu’il soit proposé à toute la population dès 60 ans, et à toutes les personnes immunodéprimées à partir de 16 ans…mais il ajoute qu’il faudra tenir compte des analyses économiques avant de prendre une décision de remboursement. En effet, ce vaccin est très cher : 170 € par dose, et il faut deux doses. Le Centre fédéral d’Expertise des Soins de Santé (KCE) rend à son tour son avis : ce prix est trop élevé par rapport aux bénéfices de santé que l’on peut attendre de ce vaccin. Si le remboursement était accordé, cette vaccination coûterait au budget des soins de santé environ 602 millions € la première année, et ensuite près de 23 millions € par an. La question est de savoir si des dépenses aussi importantes peuvent se justifier pour une maladie qui reste relativement bénigne pour la plupart des gens, surtout en cette période de restrictions budgétaires.
