Tratamiento manual de la dispepsia funcional: protocolo de fisioterapia

Abstract

Introducción: la dispepsia funcional es un trastorno frecuente en Atención Primaria y especializada, con impacto en calidad de vida y consumo de recursos. En la práctica clínica interesa comprender la evolución individual y el papel de intervenciones no farmacológicas complementarias. Objetivos: describir un caso clínico de dispepsia funcional tratado con un protocolo manual complementario, documentar la evolución clínica y de la calidad de vida, y discutir su alineación con guías actuales. Métodos: caso único, mujer de 22 años con diagnóstico de dispepsia funcional, síntomas diarios y tratamiento farmacológico estable. Se aplicó un protocolo manual de tres sesiones semanales que incluyó técnicas miofasciales paravertebrales, movilización cervical tipo POLD, relajación diafragmática, lift dorsal y arcos craneales. Se registraron intensidad sintomática, duración y frecuencia, estrés percibido, y calidad de vida específica con cuestionario validado en español. Resultados: no se observaron cambios clínicamente relevantes tras las tres sesiones. La puntuación de calidad de vida mostró variación pequeña y no significativa. El estrés aumentó coincidiendo con periodo de exámenes. El curso del caso es coherente con la variabilidad y cronicidad descritas en la literatura, y con las guías que priorizan el abordaje escalonado, la erradicación de H. pylori cuando proceda y el soporte psicoeducativo. La evidencia sobre terapia manual en dispepsia es limitada y heterogénea, por lo que su uso debe considerarse complementario y individualizado. Conclusiones: en este caso, un protocolo manual breve no modificó de forma significativa los desenlaces. Se recomiendan intervenciones multimodales, monitorización con instrumentos validados y diseños controlados en investigación.
