Abstract

La salud bucodental continúa siendo un eslabón olvidado en los cuidados de enfermería, pese a su enorme carga de enfermedad, costes y repercusión sobre la calidad de vida. La evidencia internacional ha situado las enfermedades orales —caries, periodontitis, pérdida dentaria y cáncer oral— entre las más prevalentes y socialmente desiguales, reclamando su integración plena en las políticas de salud y en la práctica clínica cotidiana de los cuidados profesionales. Más allá de la cavidad oral, la periodontitis se asocia de manera independiente con la enfermedad cardiovascular; los mecanismos plausibles incluyen inflamación sistémica, bacteriemias transitorias y disfunción endotelial. El consenso EFP–World Heart Federation sintetiza estas evidencias y formula recomendaciones clínicas concretas para equipos multiprofesionales, en las que enfermería puede y debe desempeñar un papel activo en el cribado, la educación y la derivación coordinada. En personas con diabetes tipo 2, una revisión sistemática y metanálisis de ensayos aleatorizados muestra que el tratamiento periodontal mejora el control glucémico, con reducciones clínicamente relevantes de la HbA1c—especialmente cuando el punto de partida es elevado—, lo que refuerza la oportunidad de integrar el consejo y la vigilancia bucodental en los programas de crónicos liderados por enfermería.
