Insulinoma. Informe de un caso

Abstract

Los insulinomas son tumores endocrinos del páncreas, poco frecuentes, con preferencia sobre el sexo femenino, con edad promedio en la quinta década de la vida. El cuadro clínico es prolongado y se caracteriza por neuroglicopenia, que puede involucrar la pérdida de la conciencia, crisis convulsivas, dificultad para despertar, alteraciones visuales, confusión, letargia, debilidad, conducta anormal, lenguaje farfullado, ganancia de peso y déficit motores transitorios, así como sín- tomas autonómicos caracterizados por tremor, diaforesis y palpitaciones. Los criterios actuales para determinar la pre- sencia de un insulinoma son 1) documentar niveles de glu- cosa en sangre menores de 40 mg/dL, 2) niveles de insulina concomitantes iguales o mayores a 6 muU/mL, 3) niveles elevados del péptido-C (> 200 pmo/L, y 4) determinación de sulfonilureas negativos. El método diagnóstico más pre- ciso es el ultrasonido transoperatorio. El tratamiento defini- tivo es posible a través de la cirugía con muy baja morbi- mortalidad. La evolución del cuadro es en la mayoría benig- no con un pronóstico favorable.
