Abstract

Antecedentes. A partir de Hodge, muchos autores han intentado mejorar la tasa diagnóstica de cán- cer de próstata mediante la toma de biopsias guiadas por ultrasonido transrectal de próstata (USGTRP). Ohjetivo. Determinar si la toma de biopsia puede ser optimizada para mejorar el diagnóstico de cáncer de próstata. Método. Un total de 113 pacientes fueron sometidos a USGTRP con toma de biopsias. El grupo 1 de 49 pacientes fue sometido a toma de biopsias en sextantes y el grupo 2 de 64 pacientes fue sometido a toma de biopsias sistematizadas modi- ficando la próstata en 3 sitios (derecho, centro e iz- quierdo) con toma de 4 a 6 cilindros por sitio y 3 biop- sias adicionales al encontrar una lesión hipoecoica por USGTRP. Resultados. Se realizó diagnóstico de cán- cer de próstata en 23 pacientes del grupo 1 (tasa de detección del 35%) y en 16 pacientes del grupo 2 (tasa de detección del 32%) (P > 0.05). Conclusiones. No se demostró un aumento en la sensibilidad y especifici- dad diagnóstica de cáncer de próstata al utilizar nues- tro protocolo de biopsias sistematizadas ni al realizar la toma de 3 biopsias adicionales sobre las áreas hi- poecoicas encontradas por USGTRP.
