Achado incidental de taquicardia supraventricular com evolução para Flutter atrial após cardioversão elétrica em neonato

Abstract

Arritmias cardíacas em recém-nascidos, embora raras, estão frequentemente associadas à prematuridade e cardiopatias congênitas. O Flutter Atrial, uma taquiarritmia com ritmo atrial contínuo e elevado, ocorre em cerca de 1% dos casos pediátricos e geralmente em corações previamente saudáveis. A Taquicardia Supraventricular (TSV), uma anomalia na condução elétrica acima dos ventrículos, pode causar sintomas como dificuldade na alimentação e irritabilidade. Em neonatos, a TSV pode ser assintomática por longos períodos, mas pode levar a descompensação hemodinâmica súbita. O tratamento inclui medicamentos antiarrítmicos, com adenosina sendo a primeira linha. Em casos refratários, como o descrito, Amiodarona e cardioversão elétrica sincronizada podem ser necessárias. O caso relatado descreve uma recém-nascida com TSV e Flutter Atrial persistente, tratada com adenosina, Amiodarona e cardioversão elétrica sincronizada, resultando na normalização do ritmo cardíaco. A paciente evoluiu bem com Amiodarona oral, sem sequelas. A combinação de medicamentos e cardioversão elétrica é eficaz para tratar arritmias neonatais complexas.
