Abstract

Consideraciones previas En el abordaje de la hipercolesterolemia como medida de prevención primaria se propone una estrategia para actuar en la población de riesgo elevado estimando la probabilidad de desarrollar un episodio coronario (RC) o cardiovascular (RCV). El RC puede abarcar la totalidad de episodios coronarios (IAM mortal o no, muerte súbita, infarto silente, angina de pecho) o ceñirse solo a los episodios más objetivables, es decir, excluyendo angina e infarto silente. En el primer caso, el RC se denomina riesgo coronario total (por ejemplo, las tablas de Framingham y las adaptadas de REGICOR a la población española), y en el segundo caso, riesgo coronario restringido. El RCV puede ceñirse solo a los episodios mortales o al conjunto de estos episodios. La función SCORE2 estima el riesgo de enfermedad coronaria y enfermedad cerebrovascular mortal y no mortal, muerte por IC o muerte súbita en individuos de 40 a 69 años. En nuestro entorno se propone utilizar la versión de SCORE2 para poblaciones de bajo riesgo. Se debe actuar sobre otros FRCV no controlados: prioritario dejar de fumar y controlar la PA.
