Abstract

Consideraciones previas Objetivos terapéuticos: prevenir las complicaciones macrovasculares y microvasculares, evitar las hipoglucemias y mantener la calidad de vida.. La intensidad de las actuaciones dependerá de la edad, años de evolución de la DM, comorbilidad, esperanza de vida y complicaciones microvasculares y macrovasculares. Los objetivos y las intervenciones deben ser compartidos con la/el paciente para lograr la máxima adherencia al tratamiento, abordando todos los FRC modificables. Objetivos glucémicos: la HbA1C es el indicador principal y predice el riesgo de complicaciones. Se debe solicitar dos veces al año, y si hay cambios, se recomienda evaluar a los 3-4 meses (GR C). HbA1C 6,5-7%: en pacientes con buen estado de salud. HbA1C 7-8%: en pacientes con riesgo de hipoglucemias (diabetes de larga duración, IRC, polimedicados o déficit cognitivo) (GR B). En pacientes >75-80 años, clínica de hipoglucemias, esperanza de vida <10 años, cáncer, demencias, IRC, ICC, EPOC grave o institucionalizados (residencias), es razonable tener objetivos menos estrictos.
