Abstract

Siguen vigentes las recomendaciones sobre el abordaje práctico del enfermo que ha presentado una posible convulsión y no está diagnosticado previamente de epilepsia9,10. Ante la sospecha de una convulsión, es necesario obtener una historia detallada del cuadro clínico a través del testimonio de los posibles testigos de esta. La exploración física debe descartar aquellas causas agudas que requieren una intervención urgente: fiebre y rigidez de nuca, para descartar meningitis; heridas o golpes en la cabeza, para descartar origen traumático o focalidad neurológica, que indicarían lesión en el sistema nervioso central. Si el cuadro clínico de la convulsión orienta a un cuadro no epiléptico, debe valorarse la posibilidad de un cuadro vasovagal o de un síncope cardiogénico que hayan causado una hipoxia cerebral. Si se confirma clínicamente la convulsión sin causa claramente identificable, se debe sospechar la posibilidad de que el paciente padezca epilepsia y se trate de un primer episodio convulsivo que debe valorar un médico con experiencia. Cabe tener en cuenta la posibilidad de causa epiléptica ante sensaciones extrañas o comportamientos anómalos sin otra causa explicativa, aunque es muy poco frecuente.
