Abstract

El sangrado rectal en AP precisa ser evaluado9,10 con anamnesis y exploración física para conocer la estabilidad hemodinámica y descartar masa abdominal/rectal. Solicitar hemograma y sangre oculta en heces. Las causas benignas de sangrado rectal (hemorroides, fisura, divertículos...) son más frecuentes que las neoplasias, pero pueden coexistir y retrasar el diagnóstico. Los síntomas y signos aislados en un paciente con sangrado rectal aumentan poco la probabilidad de cáncer colorrectal; la combinación de síntomas, signos y pruebas complementarias es un predictor más poderoso. La suma de heces más blandas y frecuentes en un paciente > 60 años sin síntomas perianales incrementa significativamente la probabilidad de cáncer colorrectal. El papel del clínico es usar esas combinaciones considerando los hallazgos positivos y negativos.
