Comparative Effects of Mobilization with Movement Versus Spencer Technique Combined with Scapular Stabilization Exercises on Pain, Range of Motion, and Functional Disability in Patients with Adhesive Capsulitis

Abstract

Background: Adhesive capsulitis is a painful shoulder condition causing progressive limitation of movement and functional impairment. Both mobilization with movement and the Spencer technique are commonly used in physiotherapy practice. Objective: To compare the effects of mobilization with movement and the Spencer technique, both combined with scapular stabilization exercises, on pain intensity, shoulder range of motion, and functional disability in individuals with adhesive capsulitis. Methodology: A randomized controlled trial was conducted at the Government General Hospital, Neshatabad, Peshawar. Group A received Mobilization with Movement combined with scapular stabilization exercises, while Group B received the Spencer technique with the same exercise program. Participants aged between 40 and 65 years with shoulder pain for at least three months, restricted passive external rotation greater than 30% compared with the unaffected side, and a pain score of 4 or above on the Visual Analog Scale were included in the study. Participants with a history of shoulder fracture, dislocation, surgery, rotator cuff tear, neurological or inflammatory joint disease, or recent corticosteroid injection were excluded. Outcomes included pain intensity using the Visual Analog Scale, functional disability through the Shoulder Pain and Disability Index, and shoulder range of motion assessed with a universal goniometer. Data were analyzed using SPSS version 26.0 with paired and independent t-tests and mixed ANOVA. Results: Both groups demonstrated significant improvements in pain, shoulder mobility, and functional disability after treatment (p<0.001). However, the mobilization with the movement group showed greater improvement in all outcomes. Pain scores improved from 7.2±0.8 to 2.3±0.6 in Group A and from 7.3±0.7 to 3.5±0.7 in Group B. Disability Index scores also improved more markedly in Group A compared to Group B. Conclusion: Both interventions were effective, but Mobilization with Movement combined with scapular stabilization exercises produced superior outcomes in reducing pain, improving range of motion, and enhancing functional recovery in adhesive capsulitis.
