Abstract

Kemajuan teknologi bedah oftalmologi memungkinkan banyak prosedur dilakukan secara one-day care. Seiring dengan meningkatnya populasi lanjut usia dan bayi prematur, permintaan akan teknik anestesia yang lebih aman dan minim efek sistemik terus bertambah. Hal ini menuntut spesialis anestesiologi untuk terus menyempurnakan teknik anestesia, dengan mempertimbangkan kebutuhan pasien, kondisi pembedahan, dan preferensi operator. Pilihan anestesia untuk bedah mata mencakup anestesia topikal, regional, sedasi, umum, atau kombinasi. Anestesia lokoregional banyak digunakan karena prosedur oftalmologi umumnya singkat dan bersifat ambulatori, memungkinkan pemulihan cepat. Teknik ini juga bermanfaat bagi pasien berisiko tinggi dan tidak memerlukan puasa. Beberapa teknik lokoregional yang digunakan antara lain blok retrobulbar, peribulbar, dan subtenon. Blok retrobulbar mulai ditinggalkan karena risiko komplikasi yang lebih tinggi, sementara blok peribulbar dan subtenon lebih disukai karena efektivitas dan keamanannya. Blok subtenon menggunakan kanula tumpul, mengurangi risiko perforasi bola mata dan toksisitas obat. Teknik ini lebih aman bagi pasien dengan terapi antikoagulan, meskipun dapat menyebabkan perdarahan subkonjungtiva atau kemosis. Studi terbaru menunjukkan superioritas blok subtenon dibandingkan peribulbar dalam pembedahan vitreoretina, meskipun adopsi teknik ini masih menghadapi tantangan. Kurangnya familiaritas, hambatan operasional, dan biaya alat menjadi kendala utama. Di Indonesia, perkembangan anestesia oftalmik semakin pesat, didukung oleh PERDATIN dan peningkatan pelatihan serta penelitian. Pemahaman mendalam mengenai berbagai teknik anestesia sangat penting agar anestesiolog dapat memilih metode yang optimal bagi setiap pasien, meningkatkan efisiensi layanan, dan menjamin keselamatan pasien bedah mata.
