Abstract

Radioterapija u pedijatrijskoj onkologiji donosi specifične izazove i razlike u odnosu na terapiju zračenjem odraslih pacijenata. Djeca su najosjetljivija skupina pacijenata jer su njihovi organi i tkiva i dalje u razvoju, što znači da ionizirajuće zračenje ima puno veći utjecaj i štetnost na zdrava tkiva kod djece nego kod odraslih ljudi. U pedijatrijskoj onkologiji vrlo često nailazimo na tumore velikih volumena na specifičnim lokalizacijama koji predstavljaju velik izazov u planiranju i provođenju radioterapije. Veliki je rizik zračenja i pojava sekundarnih karcinoma zbog oštećenja zdravog tkiva tijekom liječenja primarnog tumora, a novim metodama taj je rizik moguće smanjiti. Većini djece teško je ili nemoguće objasniti važnost mirovanja tijekom radioterapije, što znači da je većina djece podvrgnuta anesteziji. Svako dijete i obitelj suočeno s dijagnozom tumora prolazi kroz najteže trenutke života, stoga bi s djecom i njihovim obiteljima trebali raditi stručnjaci specijalizirani u pedijatrijskoj onkologiji i radioterapiji. Djeca se najčešće, tako i u Hrvatskoj, liječe u bolnicama i ustanovama namijenjenima odraslim ljudima. Kako bi im se olakšao proces liječenja, bitno je objasniti im cijeli proces kroz koji će prolaziti kroz oblik igre i razgovora prilagođenim dobi djeteta. Nakon dijagnoze slijedi priprema i planiranje zračenja, što obavlja liječnik onkolog u dogovoru s radiološkim tehnologom i medicinskim fizičarom. Na kraju slijedi liječenje, odnosno radioterapija, koju provode radiološki/radioterapijski tehnolozi. Uloga radiološkog/radioterapijskog tehnologa je uputiti, pripremiti i namjestiti dijete u položaj prikladan za zračenje te upravljati uređajem za radioterapiju, linearnim akceleratorom. Današnjim napretkom tehnologije i dolaskom novih metoda liječenja omogućuje se puno bolja isporuka zračenja tumorskom tkivu te puno bolja zaštita okolnog zdravog tkiva.
