Solución de sellantes de fibrina para prevenir la fuga postoperatoria en la gastrectomía vertical laparoscópica

Abstract

Los sellantes tisulares de fibrina son una opción terapéutica como método de prevención de la fuga postoperatoria en la gastrectomía vertical laparoscópica. Se analizó de forma descriptiva una serie de gastrectomías verticales de 116 pacientes. La duración media de la intervención fue de 59,43 minutos [25 – 190]. 19 pacientes (16,37 %) llevaron drenaje aspirativo en el postoperatorio. En 24 pacientes (20,68 %) se sumó sutura continua reabsorbible de la línea de grapado. La estancia media fue de 2,96 días [1 – 9]. Como complicaciones aparecieron un caso (0,86 %) de hemoperitoneo, 2 casos (1,72 %) de intolerancia digestiva en las primeras 48 horas, y 4 reingresos (3,44 %) por fuga de la línea de sutura en cardias, todos ellos realizados con endograpadora automática sin sutura de refuerzo. El empleo de sellantes de fibrina en la gastrectomía vertical laparoscópica puede ser una opción terapéutica para mejorar el índice de fuga postoperatoria.
