Abstract

Les psychoses post-ictales correspondent a l apparition soudaine d un etat delirant aigu de duree breve apres une salve de crises epileptiques. Le syndrome psychotique est de semiologie proteiforme, associant en proportions variables des symptomes thymiques, des hallucinations et des idees delirantes avec une conscience globalement preservee. Le risque de comportements dangereux auto- ou hetero-agressifs est eleve. L evolution se fait vers la regression spontanee des symptomes en une semaine en moyenne, avec ou sans traitement. Le cas clinique presente concerne un patient epileptique age de 26ans, hospitalise au service de psychiatrie de Casablanca pour trouble du comportement a type d hetero-agressivite , le patient a presente un tableau delirant aigu, sans desorganisation conceptuelle , pas de symptomatologie thymique maniaque ou depressive, le traitement instaure etait a base d haloperidol et diazepam . Un avis neurologique a ete demande, le diagnostic evoque est une psychose post-ictale, le traitement antiepileptique a ete ajuste, l evolution sous traitement fut favorable avec nettoyage totale de la symptomatologie psychotique au bout de 4jours. Le syndrome de PPI, s il est de bon pronostic a court terme, peut potentiellement evoluer vers d autres troubles psychiatriques de moins bon pronostic. Il est donc important a connaitre pour le clinicien, d autant que son traitement apparait relativement aise par rapport a celui d autres troubles psychotiques.
