PRISE EN CHARGE CHIRURGICALE DES STENOSES ANTROPYLORIQUES: A PROPOS DE 61 CAS

Abstract

La prise en charge des stenoses antropyloriques depend du stade evolutifde la stenose pouvant aller dun traitement endoscopique pour les stades initiaux jusqua la resection chirurgicale pour les stades plus tardifs. La disponibilite dun plateau technique performant constitue classiquement une des principales conditions pour la reussite de cetteattitude. Nous essayons detudier les differents aspects epidemiologiques, diagnostiques et therapeutiques de cetteaffection a travers les resultats dune serie de 61 malades. Une serie de 61 patients est prise en charge par le traitementoperatoire. La gravite des lesions est appreciee par les donnees cliniques et paracliniques , ainsi que la morbidite et la mortalite sont analysees. A linstar de la litterature, letiologie ulcereusereste de loin la plus frequente ce qui na pas concorde avec notre serie dont les stenoses cancereuses etaient les plus frequents avec une frequence de 59% dans notre contexte. La duree moyenne desurveillance etait de 7 jours. Unelaparotomie etait indiquee chez tous les patients. On avait note un taux de mortalite de 3.7% soit 4 cas. Il semble a partir de notre experience que la rapidite de la prise en charge pluridisciplinaire peut constituer une reference therapeutiquedans les pays occidentaux comme dans les pays en voie de developpement,sureetjustifiee.
