CAMBIOS HISTOPATOLOGICOS EN PACIENTES CON HERNIA DE PARED ABDOMINAL Y SU ASOCIACION CON COLAGENOSIS

Abstract

Una hernia es recidivante cuando aparece una protuberancia nueva en donde se opero previamente. Del total de plastias en Mexico, 25% son cirugias por recidiva de hernias inguinales. Existen factores que influyen en la recidiva de hernia inguinales: Excesiva tension de sutura, elevaciones de la presion intraabdominal, defectos en el metabolismo de la colagena, comorbilidades, factores locales del sitio quirurgico, tecnica quirurgica y experiencia del cirujano. De un total de 39 pacientes se encontraron diferencias en el porcentaje de colagena en muestras de aponeurosis de linea media con antecedente de hernia respectivamente (p=0.001). Por lo que, existen diferencias significativas en las variables de tipo hernia de pared, colagenosis, asi como en las caracteristicas macroscopicas de la aponeurosis. De igual manera se encontraron diferencias en el tipo de sexo, predominando el masculino, y una prevalencia de > 60 anos. Entre IMC y disminucion de colageno no se establecio una correlacion positiva. Objetivo: Evaluar y analizar los resultados obtenidos por microscopia optica de la estructura de las fibras de colageno en pacientes con y sin patologia herniaria. Material y metodos : Se realizoel estudio histopatologico deaponeurosis de linea mediaen pacientes con diagnostico de hernia de la pared abdominal, el grupo de comparacion fueron pacientes intervenidosquirurgicamente sin evidencia o antecedente de defecto herniario. Se realizo el analisis descriptivo e inferencial con el fin de comparar si existiancambios en el porcentaje de colageno en ambos grupos. Resultados: En pacientes con presencia de hernia de pared evidenciada clinicamente, el recambio de los colagenos de la matriz intesticial ( p 0.05) disminuyo en comparacion con los controles. Conclusiones: Los pacientes que presentan una hernia de pared abdominal concomitantemente presentara otra hernia de cualquier otra region de la misma(P 0.036), por lo que se sugiere en la practica diaria la busqueda intencionada de otro defecto herniario y de ser posible realizar el mismo realizar la intervencion en el mismo tiempo quirurgico, asi, de esta manera disminuir los costos y riesgos inherentes al procedimiento.
