Abstract

Le garrot est un dispositif destine a interrompre la circulation du sang dans un membre en comprimant les vaisseaux sanguins contre les os par un serrage peripherique facilitant ainsi la visibilite des vaisseaux lors des prelevements sanguins,cependant le delai pour la levee dun garrot sur un membre est limite, tout retard, surtout apres la 3eme heure expose a un risque damputation de celui-ci. Notre objectif a ete de rapporter deux cas de gangrene ischemique de membre par oubli dun garrot ou utilisation repeteelors des prelevements sanguins, afin dinterpeler le personnel soignant sur ses dangers surtout chez des patients fragiles comme les nouveau-nes. Il sagissait dedeuxnouveau-nes dont le premier hospitalise a une semaine de vieau service de neonatologie du centre hospitalier mere et enfant de Nouakchott pour une deshydratationmoderee et perte de poids, et le deuxieme etait hospitalise au même service a la premiere 24h de vie pour detresse respiratoire et infection materno-fœtale precoce, pratiquement les deux bebes avaient developpe une tumefaction du membre inferieur gauche. Dans leurs antecedents, on a note une pose repetee de garrot pour prelevement de sang. Lexamen avait retrouve un œdeme diffus associe a un sphacele du membre inferieur qui etait plus marque dans le premier cas en même temps quune abolition des pouls pedieux ainsi que de la sensibilite dans les 2 cas. Chez le deuxieme patient les signes etaient attenues. Le diagnostic de gangrene ischemique de membre a ete retenu dans tous les cas. La biologie realisee etait normale. En urgence, il a ete realise une amputation trans-tibialedans le premier cas apres echec dun traitement conservateuret un debridement avec un traitement medical chez le deuxieme patient. Levolution etait grave pour le premier malade, marque par un choc septique puis deces 48 heures en post amputation, cependant levolution etait simple et favorable dans le deuxieme cas sans recours a lamputation. La gangrene seche iatrogene de membre par garrot en milieu hospitalier ne devrait pas se concevoir. Cela passe par la rigueur dans ladministration des soins et une surveillance reguliere et attentive des patients.
