A general predictor of clinical event rates in patients that are not undergoing an acute injury is an unsolved problem. It is a hard, deep tech problem, beyond the resources of any known organisation however large. Its solution, if at all possible, might however be required in order to achieve another substantial increase in life expectancy. This introduction attempts to outline the general research direction towards that goal, the main challenges and possible approaches.
