Abstract

Introducción: el quiste de Thornwaldt es una condición rara de la línea media de la nasofaringe. Se origina a partir de restos de la notocorda en el período embrionario, cerca del músculo constrictor superior de la faringe y envuelto por la mucosa de la nasofaringe. Su diagnóstico es clínico e imagenológico si manifiesta síntomas, y secundario durante la búsqueda de otra afección. Objetivo: presentar el caso de una paciente femenina con quiste de Thornwaldt asintomático, encontrado de manera incidental en el servicio de imagenología del Hospital General Provincial Docente “Dr. Antonio Luaces Iraola” de Ciego de Ávila. Presentación del caso: mujer de 47 años de edad, con antecedentes de dolor cervical y posterior irradiación al hombro y brazos; con diagnóstico radiológico de artropatía degenerativa cervical, trastorno en la movilidad del cuello, parestesia y debilidad en los miembros superiores. En las imágenes de la columna cervical se observaron estenosis de los agujeros de conjunción y complejo disco-osteofito causante de indentación en el saco tecal y el ligamento longitudinal posterior (diámetro anteroposterior de 8 mm). Una imagen redondeada de 12 mm en la línea media de la pared posterior de la nasofaringe, de bordes bien definidos y aspecto líquido, relacionable a un quiste de Thornwaldt. Conclusiones: la paciente presentó alteraciones de la columna cervical que requirieron exámenes imagenológicos, en los cuales se observó el quiste como hallazgo incidental. Es importante el diagnóstico oportuno de esta afección rara, para evitar daños a la calidad de vida de los afectados por tratamientos invasivos.
