Bacteriemias en pacientes oncológicos.

Abstract

Introducción: las características clínicas y microbiológicas de la bacteriemia en pacientes con cáncerpueden variar dependiendo del sitio y lugar de adquisición de la infección. El objetivo de este estudioes conocer los principales microorganismos responsables de bacteriemias presentadas en pacientesoncológicos, identificar los factores de riesgo para infección por microorganismos multidrogorresistentes,y con base en ello elegir la terapia antibiótica empírica inicial.Materiales y métodos: estudio descriptivo, prospectivo de episodios de bacteriemia en pacientesoncológicos atendidos en un centro de referencia regional de tercer nivel de atención durante 12 meses.Resultados: se incluyeron en total 146 episodios de bacteriemias. Los bacilos gram negativos fueronlos microorganismos predominantes, representando la Escherichia coli 30 episodios (21%). Laincidencia de E. coli y Klebsiella pneumoniae, productoras de betalactamasa de espectro extendido,fue del 20%. La incidencia de K. pneumoniae, productora de carbapenemasas, es menor al 10%. El43% de los pacientes presentaron diagnóstico de neoplasias hematológicas, mientras que el 57%fueron tumores sólidos. Solo el 12% tuvo episodios de neutropenia febril. El 66% de los pacientesinformó como antecedente haber estado hospitalizado, sin embargo, solo el 7% reporta estanciaprevia en UCI. La mortalidad fue del 43%.Conclusiones: en la terapia antibiótica empírica inicial en cuadros de infecciones graves/sepsis enpacientes oncológicos, debe considerarse el uso de carbapenems, dado que el 20% de las enterobacteriasson productoras de BLEE. La terapia combinada con colista y aminoglucósidos se debe teneren cuenta en pacientes con factores de riesgo, pero debe ser ajustada con resultados definitivos delos cultivos.
