Abstract

La hipereosinofilia se encuentra presente en un conjunto de trastornos con producción excesiva de eosinófilos, capaz de generar daño a órganos y sistemas. El estudio genético y molecular de estas entidades ha permitido la clasificación de un grupo de neoplasias mieloides o linfoides de evolución clínica variable. En neoplasias mieloides con hipereosinofilia, la afectación pulmonar es relativamente frecuente, sin embargo, la presentación con cuadro de síndrome de dificultad respiratoria aguda (SDRA) es muy inusual. En este artículo exponemos el caso de un paciente masculino con rápida instauración de síntomas respiratorios, hepatoesplenomegalia, anemia, leucocitosis con eosinofilia y detección de reordenamiento FIP1L1-PDGFRA, al que se le estableció diagnóstico bajo criterios OMS de neoplasia mieloide con eosinofilia y reordenamiento PDGFRA. Se inició tratamiento con imatinib, logrando rápida resolución de cuadro hematológico y respiratorio.
