Tocilizumab en casos severos de COVID-19 ¿El tiempo importa?

Abstract

Introducción: El objetivo fue evaluar si el momento de la administración del tocilizumab influyó en el desenlace de los pacientes con enfermedad severa por COVID-19. Materiales y métodos: Se realizó un estudio transversal que incluyó a 56 pacientes con diagnóstico confirmado de enfermedad pulmonar severa por COVID-19 tratados con corticoesteroides y tocilizumab. Se realizó una correlación biserial para evaluar la relación entre días sintomáticos previos al tratamiento con el estado de supervivencia, y una prueba de chi cuadrado para valorar la asociación entre el dosaje de corticoesteroides/tocilizumab con el estado de supervivencia. Resultados: Se encontró una correlación negativa estadísticamente significativa entre los días previos a la administración de tocilizumab y el estatus de supervivencia (rpb = -.290, n = 56, p = .030). La asociación entre la dosis de tocilizumab recibida y el estatus de supervivencia no fue estadísticamente significativa (χ2(5) = 3.566, p = .600). Conclusiones: En el presente estudio determinó que la administración temprana de tocilizumab estaba asociada a una mayor posibilidad de sobrevivir, una observación que no pudo ser reproducida en el caso de los corticoides sistémicos. Es necesaria la realización de estudios adicionales que permitan identificar el tipo de paciente que podría beneficiarse del tratamiento con tocilizumab, y si el tiempo hasta la administración representa un factor importante a tener en consideración.
