Streptococcal Toxic Shock Syndrome: Two Case Reports

Abstract

Streptokoksik toksik sok sendromu gram pozitif kok olan A grubu streptokoklarin (AGS) olusturdugu klinik bir tablodur. Genellikle sporadik olarak gozlenen streptokoksik toksik sok sendromu olgularinin zaman zaman huzurevleri, hastane gibi toplu yasanilan ortamalarda salginlar yaptigi bilinmektedir. Streptokoklarin giris yolu olgularin yaklasik yarisinda farenks, cilt ve vajinadir. Etken siklikla faranjit, impetigo, erizipel ve lokalize selulit gibi yuzeysel deri enfeksiyonlarini da iceren yumusak doku enfeksiyonlarina neden olmaktadir (1, 2). AGS’da M proteini onemli bir virulans belirtecidir ve M proteini icermeyen zincirler daha az virulandirlar. M proteini filamentoz bir protein olup, antifagozitos ozellikleri olan bir hucre membran proteinidir. Toksik sok sendromu ve multipl organ yetmezligin (MOY)’de en sik tespit edilen tipler M 1, 3, 12 ve 28’dir. Streptokokkal pirojenik ekzotoksinler AGS’lar tarafindan meydana getirilmektedir. Farkli uc ayri antijenik sekiller (A, B, C) tanimlanmistir. Bu toksinler sitotoksiteyi, pirojeniteyi ve endotoksinin letal etkilerini arttirici etki gosterirler. Pirojenik eksotoksin A (SPEA) ve/veya B (SPEB) agir AGS enfeksiyonlarinin cogunda bulunmustur (3). Bu pirojenik ekzotoksinlerin ates, endotoksine duyarlik artisi, IgM antikor sentezinin baskilanmasi ve superantijen rolu oynamak gibi ozellikleri vardir. Bu toksinler T hucre cevabini uyarabilir. Net etki monosit kokenli sitokinlerin (TNF α, IL-1β, IL-6) ve lenfokinlerin (TNF β, IL-2, IFN γ) uyarilmasi seklindedir. Ozet
