Abstract

Hareket bozukluğu olan hastaları tedavi etmek için rezeksiyon, lezyon oluşturma, stimülasyon gibi çeşitli cerrahi yaklaşımlar kullanılmıştır. Bazal ganglionlar veya etrafındaki yapıların uyarımı yöntemine derin beyin stimulasyonu (DBS) ismi verilir. Kendisinden önce uygulanan lezyon cerrahisi sayesinde gelişmiştir. DBS’nin lezyon oluşturma prosedürlerine kıyasla bazı avantajları vardır: i. yan etki olasılığı daha düşüktür, ii. stimülasyon parametreleri değiştirilerek etki kontrol edilebilir, iii. iki yanlı uygulanmasının daha güvenli olduğu gösterilmiştir, iv. intraoperatif kayıtlar bazal nükleus devrelerinin daha iyi anlaşılmasını sağlayan araştırma aracı haline gelmiştir. Bunlara ilave olarak sıklıkla geri dönüşümsüz hasar vermediği kabul edilmektedir. Bu bölümde hareket bozukluklarında başlıca DBS endikasyonları, yan etkiler, cerrahi işlem ve prognozdan bahsedilecektir
