Abstract

Bradikinezinin ana bileşeni oluşturduğu parkinsonizm ile karakterize hipokinetik hareket bozuklukları Parkinson hastalığı ile Progresif Supranükleer Paralizi, Multipl Sistem Atrofi, Kortikobazal Dejenerasyon ve Lewy Cisimli Demanstan oluşan Atipik Parkinsonizm Sendromlarından oluşan dejeneratif hastalıklar ile sekonder parkinsonizm tablolarını içerir. Bradikinezinin patofizyolojisini oluşturan kortiko-striato-talamo-kortikal devrenin işlev bozukluğu Parkinson hastalığı başta olmak üzere tüm hipokinetik hareket bozukluklarında benzer olsa da patolojik bulgular ve patogenez sekonder ve atipik parkinsonizm sendromlarında farklıdır. Klinik belirti ve bulguların birlikteliği ile zaman içinde yerleşim biçimi, seyri, eşlik eden hareket bozukluğu dışındaki belirtiler, başta yapısal ve işlevsel görüntüleme olmak üzere laboratuvar bulguları ve dopaminerjik tedaviye yanıt parkinsonizm ile karakterize patolojik durumların klinik tanısının temelini oluşturur. Bu bölümde parkinsonizm sendromlarının ayırıcı tanısı ve semptomatik tedavi seçeneklerinden sözedilecektir.
